Filing a bankruptcy or a proposal
requires information to be given to the trustee. The
following forms were developed to assist the trustee
in assessing your financial situation. Please print
off the forms and try to complete them so that we can
be better prepared for your assessment. If you have
questions regarding the forms, we will help you complete
the forms at the initial interview. The forms may be
faxed to us, dropped off at our offices before your
meeting with us or forwarded to us by email at
info@billmcculloch.com
or brought with you to the initial meeting.
If you send the forms to us prior
to the meeting we will review your information and
contact you to set up the intial appointment.
If you have made an appointment for
an interview, try to bring the completed forms with
you at the interview. This will help us to understand
your situation fully and plan an effective course of
action for you.

Monthly Statement of Income and Expenses |
This form is required during
bankruptcy. It has two purposes: monitor income
to determine that payments are made in accordance
with the Superintendent’s guidelines, and
provide you with a history of where your money
goes as you spend it. It will provide invaluable
information to assist you in developing future
plans. It is a good idea to use this form to
keep track of your expenses even if you file
a proposal. People with credit card problems
need to develop a history of cash expenditures
so that they can understand how their income
is spent.
